Include any recent injuries or surgeries, chronic illnesses, medication with dosage amount, allergies and special instructions. Oral and topical medications must be provided in the professionally-labeled original prescription bottles unless other arrangements are made in advance.

_____ SAFETY PROTOCOLS: Any dog in heat, exhibiting symptoms of illness, injury, or any life-threatening or contagious condition (sneezing, coughing, wheezing, runny eyes or nose, vomiting, lethargy, diarrhea, loss of appetite, limping, canine distemper, parvo, rabies etc.) will be excused from future services until such illness or injury has been remedied. Your dog must be up-to-date on vaccinations and wearing identification/rabies tags attached to a secure, flat, nylon or leather collar. Your dog must be on leash upon arrival and departure.
_____ LIABILITY: At FIND YOUR INNER DOG, LLC, we do our utmost to ensure a safe environment for all guests, humans and dogs. However, given the unpredictable nature of dogs and certain humans, an interaction could take place which may result in damage to property, illness, injury or death. FIND YOUR INNER DOG, LLC, its owner, employees, or volunteers are not responsible for any damages, injury, illness, or death. By signing and submitting this form, you, the owner, release FIND YOUR INNER DOG, LLC, including its owner, assistants, participants, spectators and all land owners, from all liability and any damages, illnesses, injuries or deaths which may arise now or in the future directly or indirectly, as a result of you, your dog, or anyone under your care, participating in activities or services provided by FIND YOUR INNER DOG, LLC and assumes responsibility for any such damage, exposure, and medical or veterinary costs.
_____RECORDS: Current vaccinations records, and proof of spay/neuter (from your veterinarian) must be on file at time of service. Please scan or take a picture of these documents and send them to email@example.com prior to drop-off. We will not admit any dog without proper identification/rabies tags, vaccination records, spay/neuter documentation, and signed agreements.
_____TRAINING METHODS & TOOLS: Training tools include, but are not limited to: flat buckle collar, prong collar, slip lead, e-collar/remote training collar, pet convincer, muzzle, and dominant dog collar. By signing and submitting this form, you, the owner, understand that any and all of these tools may be used during the training process and agree to their safe, responsible use. (N/A to B&B clients)
_____TRAINING: FIND YOUR INNER DOG, LLC cannot guarantee your dog will maintain its new behavior and state of mind in the future once your dog is in your care even though the behavior was corrected or taught during training. (N/A to B&B clients)
_____REHAB & BOARDING: We feed adult dogs two-times/day. If your dog requires a special diet, the owner is responsible for preparing it ahead of time, portioned out for each meal (AM/PM). Do NOT bring any water bowls, toys, bones or treats.
_____ABANDONMENT: Any dog left after 24 hours of the scheduled pickup without contact from the owner is considered abandoned and will be surrendered to the proper authorities.
_____PAYMENT: Payment by cash, check or through PayPal is required 100% in full at time of drop-off.
_____CANCELLATION & REFUND: TRAINING - If you choose to discontinue training before receiving approximately 50% of services, a partial, prorated refund will be issued. Any time after that, refunds will not be granted.
_____REHAB/BOARDING - If you choose to cancel boarding or REHAB up to 3 days in advance, a full refund will be issued. A 50% refund will be granted to cancellations up to the day before drop off. Cancellations the day of drop off will not be refunded.
_____EMERGENCY CARE: Should your dog become ill or injured during its stay at FIND YOUR INNER DOG LLC, you grant permission for your dog to be treated by the veterinarian listed on this contract or the closest emergency animal hospital and agree to pay said veterinarian for all services rendered. Every effort will be made to contact the owner, co-owner and emergency contact prior to having your animal treated.
_____PHOTO & VIDEO RELEASE STATEMENT: I hereby grant to FIND YOUR INNER DOG, LLC the right and permission to use, reproduce, distribute, and publish photographs and video recordings made of me and/or my dog at any time during activities or services provided by FIND YOUR INNER DOG, LLC.
Vaccination Records & Proof of Spay/Neuter:
Please acknowledge you have read the following statement and have sent these documents to findyourinnerdog@.com. Current vaccination records and proof of spay/neuter (if over 6 months of age) from your veterinarian must be on file at time of service. Please scan or take a picture of these documents and attach them to a separate email with your dog's name in the subject line. We will not admit any dog without proper identification/rabies tags, vaccination records, spay/neuter documentation and signed agreements.
